Financial Terms, Payment Administration & Policy Version Control Policy

Firm Hill Cardiometabolic Center ("Firm Hill" or "the Practice") is committed to transparent financial practices, accurate patient billing, secure payment processing, and responsible administration of patient enrollment.

This policy establishes the financial terms applicable to services offered through Firm Hill and defines procedures for approving, updating, and maintaining those terms across the Practice's website, patient management systems, and payment platforms.

This policy applies to patients purchasing or requesting services through Firm Hill's website and approved external systems.

Firm Hill operates distinct financial and enrollment pathways based on the service selected.

1.A. Tebra-Managed Consultations

The following services use the Practice's approved Tebra scheduling and payment pathway:

  • Chronic Disease Management consultations.
  • General Consultation services, excluding Extended Consultation and Comprehensive New Client Intake.
  • Internal Medicine consultations.

For these services, patients are directed to the approved Tebra scheduling system. Applicable payments, billing arrangements, insurance processing, and patient financial obligations are administered according to the Practice's configuration and applicable terms within Tebra.

Firm Hill does not require separate QuickBooks payment confirmation or an FH enrollment verification code before patients access the designated Tebra scheduling pathway.

Patients remain responsible for applicable fees, copayments, deductibles, coinsurance, or other authorized charges disclosed before payment or service delivery.

2.B. Firm Hill-Managed Enrollment and Payments

The following services retain their existing approved enrollment and payment procedures:

  • Metabolic Health Membership.
  • Comprehensive Weight Management Membership.
  • Other Weight Management services.
  • Additional Services.
  • Extended Consultation.
  • Comprehensive New Client Intake.

These services may require registration, acceptance of applicable financial terms, payment through an approved payment provider, payment confirmation, and enrollment verification before access to the relevant onboarding pathway.

Firm Hill will disclose applicable service prices and material financial conditions before a patient commits to a purchase.

Financial disclosures must identify, as applicable:

  • The service being purchased.
  • The total amount payable.
  • Whether the fee is a one-time charge or covers a defined membership term.
  • The payment provider.
  • Applicable cancellation and refund conditions.
  • Whether insurance billing is available.
  • Any additional charges that may apply.

Published website prices must be consistent with the Practice's approved financial schedule and applicable payment platform.

Where an estimate is provided rather than a final patient responsibility amount, it must be clearly identified as an estimate.

For services eligible for insurance billing, Firm Hill may direct patients to Tebra for secure registration, insurance information collection, eligibility verification, scheduling, and billing.

The Practice's website may provide a preliminary insurance acceptance check based on its approved list of participating insurers and plans.

This preliminary check does not establish individual insurance eligibility, confirm benefits, guarantee claim payment, or determine final patient financial responsibility.

Actual coverage and billing determinations remain subject to verification, applicable payer requirements, and the Practice's billing policies.

Membership services remain governed by their separately approved membership payment terms unless Firm Hill expressly establishes another compliant arrangement.

Memberships are purchased for the selected contractual term under the financial conditions presented and accepted at enrollment.

Approved membership pricing is:

Metabolic Health Membership

  • Six months: $1,700.
  • Nine months: $2,525.
  • Twelve months: $3,350.

Comprehensive Weight Management Membership

  • Six months: $2,050.
  • Nine months: $3,025.
  • Twelve months: $4,000.

Memberships are prepaid for the selected term and do not automatically renew.

The membership agreement must describe the services included, duration, cancellation procedures, refund conditions, and any limitations.

Patient-initiated cancellation does not ordinarily entitle the patient to a refund under the Practice's proposed membership terms, except where applicable law or the governing agreement requires otherwise.

Where Firm Hill is unable to provide contracted services, the Practice will determine and issue any refund required for undelivered services under applicable law and the approved agreement.

No provision of this policy limits nonwaivable patient rights or legally required refunds.

For separately purchased consultations using the Firm Hill-managed payment pathway, the applicable cancellation and refund terms must be disclosed before payment.

Under the Practice's proposed financial model:

  • Patient-initiated cancellations are generally nonrefundable, subject to applicable law and the approved service agreement.
  • If Firm Hill cancels or cannot provide the contracted consultation, the patient may elect an appropriate rescheduling arrangement or receive a refund when required.
  • Duplicate charges, confirmed billing errors, and unauthorized transactions will be investigated and corrected as appropriate.
  • Payments for services administered through Tebra are subject to the applicable approved consultation terms and payment arrangements disclosed through that pathway.

Refund determinations must consider whether services were delivered, the governing agreement, and applicable legal requirements.

Firm Hill maintains a formal approval process for material financial terms.

Each approved financial policy must have:

  • A unique policy identifier.
  • A version number.
  • An effective date.
  • The identity of the approving authorized administrator.
  • The approval timestamp.
  • A retained copy of the approved policy text.
  • A record of subsequent material revisions.

Material changes to fees, refund rights, cancellation obligations, payment requirements, membership duration, or patient financial responsibilities require review and renewed approval before becoming effective.

An approval associated with an earlier policy version does not constitute approval of materially revised terms.

Nonmaterial formatting, spelling, or presentation changes may be recorded without requiring renewed substantive approval, provided the underlying financial obligations remain unchanged.

When Firm Hill transfers a service from its website-managed payment system to Tebra:

1. The service's designated payment pathway must be updated.
2. Obsolete payment approval requirements must no longer govern new transactions through the replacement pathway.
3. Any outstanding obligations or disputes arising under earlier transactions must remain subject to their applicable agreements.
4. Existing approval history must be preserved.
5. The Practice must verify that the replacement pathway provides appropriate patient disclosures and payment controls.

Changing a service's payment platform does not automatically cancel existing patient agreements or erase prior financial records.

Patients must have a reasonable opportunity to review applicable financial terms before purchasing services.

Where affirmative acceptance is required, the Practice will maintain an appropriate record of:

  • The applicable policy version.
  • The acceptance date and time.
  • The transaction or enrollment associated with acceptance.
  • The acceptance event or approved acknowledgment method.

Acceptance records must be protected against unauthorized alteration.

Financial acceptance is separate from medical informed consent. Any required telemedicine informed consent must be obtained through the Practice's approved clinical process before care is provided.

Firm Hill will maintain financial and enrollment records necessary for lawful business operations, payment reconciliation, patient rights, dispute resolution, and applicable recordkeeping obligations.

The Practice will:

  • Limit information collection to legitimate operational purposes.
  • Apply appropriate access controls.
  • Avoid unnecessary duplication of patient records between Lovable and Tebra.
  • Use approved payment and clinical systems.
  • Protect patient information in accordance with applicable privacy and security requirements.
  • Retain records according to an approved retention schedule.
  • Avoid storing complete payment card details in the website's database.

Financial and approval records must not be automatically deleted merely because a policy version has been replaced.

Firm Hill may revise its financial policies prospectively.

Material changes must be reviewed and approved before implementation.

Revised terms will not automatically alter existing prepaid membership agreements or previously completed transactions unless permitted by the governing agreement and applicable law.

Where renewed patient acceptance is required, the Practice will obtain it before applying the revised terms.

The Practice Administrator is responsible for ensuring that:

  • Published financial terms are current.
  • Applicable payment pathways are correctly configured.
  • Material changes receive proper approval.
  • Superseded policies remain archived.
  • Staff have access to the current approved financial policies.
  • Website financial representations remain consistent with the Practice's actual operations.

This policy will be reviewed periodically and whenever material changes occur in:

  • Membership offerings.
  • Consultation pricing.
  • Payment platforms.
  • Insurance billing arrangements.
  • Applicable laws or regulatory requirements.
  • Patient cancellation and refund procedures.
