Personalized, Science-Backed Care for Sustainable Metabolic Wellness

Introduction

Lasting Metabolic Health Takes a Plan

Achieving lasting metabolic health requires consistency, expert oversight, and a long-term plan tailored to the individual.

Our Metabolic Health Membership provides structured, ongoing physician-led care focused on metabolic health, prevention, and overall wellness.

Direct Physician Support

Continuous care through scheduled consultations rather than disconnected visits.

Tailored Clinical Strategy

Individualized evidence-based approaches to metabolic health, nutrition, lifestyle, and preventive care.

Structured Follow-Up

Regular consultations designed to evaluate progress and adjust the care plan when clinically appropriate.

Flexible Commitment Options

6-month, 9-month, and 12-month membership options, subject to finalized membership terms.

Conditions we manage

Metabolic, Cardiovascular & Renal Conditions

Membership care focuses on these cardiometabolic conditions. For gastrointestinal, respiratory, joint and other adult concerns, see Chronic Disease Management.

Metabolic, Endocrine & Liver Health

Metabolic Syndrome & Insulin Resistance

Assessment and management of interconnected metabolic risk factors — abdominal adiposity, elevated blood glucose, abnormal cholesterol and high blood pressure — through individualized lifestyle interventions, risk reduction and clinically appropriate treatment.

Type 2 Diabetes & Prediabetes

Physician-led evaluation and ongoing management focused on glycemic control, cardiovascular risk reduction, lifestyle modification, medication management when appropriate, and prevention of long-term complications.

Thyroid Disorders

Evaluation and management of common thyroid conditions, including hypothyroidism, hyperthyroidism and selected stable thyroid disorders, with laboratory monitoring and specialist referral when indicated.

Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD/MASH)

Assessment and management of the metabolic risk factors associated with fatty liver disease — insulin resistance, obesity, dyslipidemia and type 2 diabetes — including risk assessment, appropriate noninvasive evaluation and specialist referral when clinically indicated.

Polyendocrine Metabolic Ovarian Syndrome (PMOS)

Formerly known as polycystic ovary syndrome (PCOS). Metabolic assessment and management of insulin resistance, weight-related concerns and associated cardiometabolic risk factors in patients with PMOS.

Dyslipidemia & Lipid Disorders

Management of elevated LDL cholesterol, low HDL cholesterol, hypertriglyceridemia and related cardiovascular risk markers, with individualized risk assessment, nutrition and lifestyle counseling, and appropriate lipid-lowering therapy.

Cardiovascular & Renal Risk

Essential Hypertension

Evaluation and ongoing management of high blood pressure, including home blood pressure monitoring, medication optimization, lifestyle interventions and cardiovascular risk reduction.

Cardiovascular Risk Assessment & Primary Prevention

Individualized assessment of blood pressure, cholesterol, diabetes, smoking history, weight, family history and other relevant indicators, with an emphasis on prevention before cardiovascular disease develops.

Atherosclerosis & Vascular Risk Management

Management of the risk factors associated with atherosclerotic disease — lipid disorders, hypertension, diabetes and metabolic dysfunction. This is preventive risk management; advanced vascular disease is referred for specialist treatment.

Early-Stage Chronic Kidney Disease (CKD) Monitoring

Monitoring of selected stable, early-stage kidney disease, including kidney function assessment, blood pressure optimization, diabetes management, medication review and appropriate nephrology referral.

Services are provided based on individual clinical needs and medical appropriateness. Certain conditions may require laboratory testing, diagnostic imaging, in-person examination, specialist referral, or emergency evaluation. Telemedicine is not appropriate for all medical concerns. Firm Hill provides adult care only, for patients physically located in Texas at the time of each visit.
